- Multiple sclerosis commonly presents in young adults, who have a relapsing-remitting multiple sclerosis course. For the majority, the course of multiple sclerosis eventually becomes progressively disabling, it is called secondary progressive multiple sclerosis. Studies of multiple sclerosis natural history showed that age affects multiple sclerosis progression independent of the disease duration. You have done sophisticated MRI analysis of multiple sclerosis cases, which were similar in everything, but the age of the patients. What did your study show? And what its implications for treatment of patients with multiple sclerosis? The study attempted to control for the duration of multiple sclerosis in a group of patients from a common middle age. What it suggested is that those patients who were older, tended to have a more aggressive clinical course, associated with more profound changes, particularly in the diffuse white matter of the brain, in ways that we associate with the greater loss of axons and neurodegeneration. This in itself is consistent with quite long-standing clinical epidemiological data, which has suggested that patients show different rates of progression, depending on the age, at which they first present with multiple sclerosis. It also highlights the differences in sex ratios between those who present with multiple sclerosis first in mid-life as opposed to early [in life]. Altogether these factors suggest that there is a strong interaction between the inflammatory processes and the subsequent neurodegeneration of multiple sclerosis and aging of the brain. In an interesting way this relates to many other neurodegenerative disorders. Of course, the most common of which is Alzheimer's disease. For all of these the single greatest risk connector for those with predisposing biology is, of course, age. 10
